CYBHI Fee Schedule TPA Program Announcements

We are excited to share a Carelon Provider Update with you. Carelon has established connections with the health plans listed below for CYBHI Fee Schedule program. Effective 10/24/2025, Providers (LEAs) will no longer need to list students who have health insurance with the listed health plans on their Student Batch Registration template.

Also, effective 10/24/2025, for students with the below listed health insurance plans, Providers (LEAs) must now list the student’s health insurance plan member/subscriber ID number in the subscriber/insured ID field when submitting claims to Carelon for CYBHI Fee Schedule program services.
